Cumulus Microphysics and Climate Sensitivity
Precipitation processes in convective storms are potentially a major regulator of cloud feedback. An
unresolved issue is how the partitioning of convective condensate between precipitation-size particles that fall out of updrafts and smaller particles that are detrained to form anvil clouds will change as the climate warms. Tropical Rainfall Measuring Mission (TRMM) observations of tropical oceanic convective storms indicate higher precipitation efficiency at warmer sea surface temperature (SST) but also suggest that cumulus anvil sizes, albedos, and ice water paths become insensitive to warming at high temperatures. International Satellite Cloud Climatology Project (ISCCP) data show that instantaneous cirrus and deep convective cloud fractions are positively correlated and increase with SST except at the highest temperatures, but are sensitive to variations in large-scale vertical velocity. A simple conceptual model based on a Marshall–Palmer drop size distribution, empirical terminal velocity–particle size relationships, and assumed cumulus updraft speeds reproduces the observed tendency for detrained condensate to approach a limiting value at high SST. These results suggest that the climatic behavior of observed tropical convective clouds is intermediate between the extremes required to support the thermostat and adaptive iris hypotheses
Clinical and Genomic Characterization of Recurrent Enterococcal Bloodstream Infection in Patients With Acute Leukemia
Background. Rates and risk factors for recurrent enterococcal bloodstream infection (R-EBSI) and whether the same genetic lineage causes index EBSI and R-EBSI are unknown in patients with acute leukemia (AL) receiving chemotherapy.
Methods. Ninety-two AL patients with EBSI from 2010 to 2015 were included. Enterococcal bloodstream infection was defined by 31 positive blood cultures for Enterococcus faecium or Enterococcus faecalis and fever, hypotension, or chills. Clearance was defined by 31 negative cultures 324 hours after last positive culture and defervescence. Recurrent enterococcal bloodstream infection was defined by a positive blood culture for Enterococcus 324 hours after clearance. Categorical variables were reported as proportions and compared by the χ2 test. Continuous variables were summarized by median and interquartile range (IQR) and compared by the Wilcoxon-Mann-Whitney Test. P values \u3c.05 were considered significant. Whole-genome sequencing was performed on available paired BSI isolates from 7 patients.
Results. Twenty-four patients (26%) had 31 episodes of R-EBSI. Median time to R-EBSI (IQR) was 26 (13–50) days. Patients with R-EBSI had significantly longer durations of fever and metronidazole exposure during their index EBSI. Thirty-nine percent of E. faecium R-EBSI isolates became daptomycin-nonsusceptible Enterococcus (DNSE) following daptomycin therapy for index EBSI. Whole-genome sequencing analysis confirmed high probability of genetic relatedness of index EBSI and R-EBSI isolates for 4/7 patients.
Conclusions. Recurrent enterococcal bloodstream infection and DNSE are common in patients with AL and tend to occur within the first 30 days of index EBSI. Duration of fever and metronidazole exposure may be useful in determining risk for R-EBSI. Whole-genome sequencing analysis demonstrates that the same strain causes both EBSI and R-EBSI in some patients

The Tropical Subseasonal Variability Simulated in the NASA GISS General Circulation Model
The tropical subseasonal variability simulated by the Goddard Institute for Space Studies general circulation model, Model E2, is examined. Several versions of Model E2 were developed with changes to the convective parameterization in order to improve the simulation of the Madden-Julian oscillation (MJO). When the convective scheme is modified to have a greater fractional entrainment rate, Model E2 is able to simulate MJO-like disturbances with proper spatial and temporal scales. Increasing the rate of rain reevaporation has additional positive impacts on the simulated MJO. The improvement in MJO simulation comes at the cost of increased biases in the mean state, consistent in structure and amplitude with those found in other GCMs when tuned to have a stronger MJO. By reinitializing a relatively poor-MJO version with restart files from a relatively better-MJO version, a series of 30-day integrations is constructed to examine the impacts of the parameterization changes on the organization of tropical convection. The poor-MJO version with smaller entrainment rate has a tendency to allow convection to be activated over a broader area and to reduce the contrast between dry and wet regimes so that tropical convection becomes less organized. Besides the MJO, the number of tropical-cyclone-like vortices simulated by the model is also affected by changes in the convection scheme. The model simulates a smaller number of such storms globally with a larger entrainment rate, while the number increases significantly with a greater rain reevaporation rate

The MJO Transition from Shallow to Deep Convection in CloudSat/CALIPSO Data and GISS GCM Simulations
The relationship between convective penetration depth and tropospheric humidity is central to recent theories of the Madden–Julian oscillation (MJO). It has been suggested that general circulation models (GCMs) poorly simulate the MJO because they fail to gradually moisten the troposphere by shallow convection and simulate a slow transition to deep convection. CloudSat and Cloud–Aerosol Lidar and Infrared Pathfinder Satellite Observations (CALIPSO) data are analyzed to document the variability of convection depth and its relation to water vapor during the MJO transition from shallow to deep convection and to constrain GCM cumulus parameterizations. Composites of cloud occurrence for 10 MJO events show the following anticipated MJO cloud structure: shallow and congestus clouds in advance of the peak, deep clouds near the peak, and upper-level anvils after the peak. Cirrus clouds are also frequent in advance of the peak. The Advanced Microwave Scanning Radiometer for Earth Observing System (EOS) (AMSR-E) column water vapor (CWV) increases by ~5 mm during the shallow–deep transition phase, consistent with the idea of moisture preconditioning. Echo-top height of clouds rooted in the boundary layer increases sharply with CWV, with large variability in depth when CWV is between ~46 and 68 mm. International Satellite Cloud Climatology Project cloud classifications reproduce these climatological relationships but correctly identify congestus-dominated scenes only about half the time. A version of the Goddard Institute for Space Studies Model E2 (GISS-E2) GCM with strengthened entrainment and rain evaporation that produces MJO-like variability also reproduces the shallow–deep convection transition, including the large variability of cloud-top height at intermediate CWV values. The variability is due to small grid-scale relative humidity and lapse rate anomalies for similar values of CWV
Universal Mask Usage for Reduction of Respiratory Viral Infections After Stem Cell Transplant: A Prospective Trial
Background. Respiratory viral infections (RVIs) are frequent complications of hematopoietic stem cell transplant (HSCT). Surgical masks are a simple and inexpensive intervention that may reduce nosocomial spread
Linearization of homogeneous, nearly-isotropic cosmological models
Homogeneous, nearly-isotropic Bianchi cosmological models are considered.
Their time evolution is expressed as a complete set of non-interacting linear
modes on top of a Friedmann-Robertson-Walker background model. This connects
the extensive literature on Bianchi models with the more commonly-adopted
perturbation approach to general relativistic cosmological evolution.
Expressions for the relevant metric perturbations in familiar coordinate
systems can be extracted straightforwardly. Amongst other possibilities, this
allows for future analysis of anisotropic matter sources in a more general
geometry than usually attempted.
We discuss the geometric mechanisms by which maximal symmetry is broken in
the context of these models, shedding light on the origin of different Bianchi
types. When all relevant length-scales are super-horizon, the simplest Bianchi
I models emerge (in which anisotropic quantities appear parallel transported).
Finally we highlight the existence of arbitrarily long near-isotropic epochs
in models of general Bianchi type (including those without an exact isotropic
limit).Comment: 31 pages, 2 figures. Submitted to CQ

IVF for premature ovarian failure: first reported births using oocytes donated from a twin sister
BACKGROUND: Premature ovarian failure (POF) remains a clinically challenging entity because in vitro fertilisation (IVF) with donor oocytes is currently the only treatment known to be effective.
METHODS: A 33 year-old nulligravid patient with a normal karyotype was diagnosed with POF; she had a history of failed fertility treatments and had an elevated serum FSH (42 mIU/ml). Oocytes donated by her dizygotic twin sister were used for IVF. The donor had already completed a successful pregnancy herself and subsequently produced a total of 10 oocytes after a combined FSH/LH superovulation regime. These eggs were fertilised with sperm from the recipient\u27s husband via intracytoplasmic injection and two fresh embryos were transferred to the recipient on day three.
RESULTS: A healthy twin pregnancy resulted from IVF; two boys were delivered by caesarean section at 39 weeks\u27 gestation. Additionally, four embryos were cryopreserved for the recipient\u27s future use. The sister-donor achieved another natural pregnancy six months after oocyte retrieval, resulting in a healthy singleton delivery.
CONCLUSION: POF is believed to affect approximately 1% of reproductive age females, and POF patients with a sister who can be an oocyte donor for IVF are rare. Most such IVF patients will conceive from treatment using oocytes from an anonymous oocyte donor. This is the first report of births following sister-donor oocyte IVF in Ireland. Indeed, while sister-donor IVF has been successfully undertaken by IVF units elsewhere, this is the only known case where oocyte donation involved twin sisters. As with all types of donor gamete therapy, pre-treatment counselling is important in the circumstance of sister oocyte donation
The Need for Laboratory Measurements and Ab Initio Studies to Aid Understanding of Exoplanetary Atmospheres
We are now on a clear trajectory for improvements in exoplanet observations
that will revolutionize our ability to characterize their atmospheric
structure, composition, and circulation, from gas giants to rocky planets.
However, exoplanet atmospheric models capable of interpreting the upcoming
observations are often limited by insufficiencies in the laboratory and
theoretical data that serve as critical inputs to atmospheric physical and
chemical tools. Here we provide an up-to-date and condensed description of
areas where laboratory and/or ab initio investigations could fill critical gaps
in our ability to model exoplanet atmospheric opacities, clouds, and chemistry,
building off a larger 2016 white paper, and endorsed by the NAS Exoplanet
Science Strategy report. Now is the ideal time for progress in these areas, but
this progress requires better access to, understanding of, and training in the
production of spectroscopic data as well as a better insight into chemical
reaction kinetics both thermal and radiation-induced at a broad range of
temperatures. Given that most published efforts have emphasized relatively
Earth-like conditions, we can expect significant and enlightening discoveries
as emphasis moves to the exotic atmospheres of exoplanets.Comment: Submitted as an Astro2020 Science White Pape
